Thank you for your interest in taking part in the Human Fertilisation and Embryology Authority’s (HFEA) national patient survey.

As well as inspecting and licensing clinics, the HFEA provides unbiased and evidence-based information on fertility treatment.

This survey will look at experiences of fertility treatment in the UK and is open to patients, partners, surrogates, and intended parents who are either currently undergoing treatment, or who have undergone treatment in the past five years.

It will cover things like your most recent round of treatment; the information or support sources you used; the use of donor eggs, sperm, or embryos in your treatment and any additional tests, treatments, or technologies.

This survey should take around 15-20 minutes to complete.

Information on emotional support is avaliable on the HFEA website, or via your clinic.
Your participation
Your participation is voluntary, and you can withdraw at any time before completing the survey by closing the window. Once your answers are submitted, we will be unable to withdraw your response. Questions marked with an asterisk are required and must be completed in order to submit your response.

This survey is designed to be anonymous so do not include any personal information in your responses. Any personal data submitted will be removed. Comments you provide may be used in reports or documents we produce and publish online. To find out more about how the HFEA manages personal information, visit our Privacy Policy.
